This form is used to report non-emergency issues to the City of Cold Lake. Please choose from the options below, explain the nature of the issue and provide as many details as possible. The information will be forwarded to the appropriate department for follow up. Please note, submissions containing inappropriate or profane language will be immediately discarded.

Do not use this form:

To report an urgent public safety matter, crime in progress or medical emergency. Call 911.
To report an after hours public works emergency such as a water main break, sewage backup, or traffic light outage. Call 780-207-0173. During regular business hours, call 780-594-4496.
To report a pothole. Use the Report a Pothole tool.
To report a stray animals, please call Municipal Enforcement at 780-594-1345
